If you are having trouble logging on to PERA's Employer Reporting and Information System, you should consider the following:

The login requires both a valid User ID and Password.
The User ID is assigned to you by PERA.
The Password must be at least 8 characters in length and must contain one Upper Case and one Number.
After 3 consecutive unsuccessful attempts to login, your Password is suspended.
If you are unable to login because your password has been suspended, contact the System Administrator at your governmental agency. If you are the System Administrator, please call PERA (1-888-892-7372 or 651-296-3636) for assistance.

This site requires a Secured Socket Layer (SSL) compliant browser.
Windows Users: We recommend using Mozilla Firefox, Microsoft Internet Explorer 6, Netscape Version 7.
Apple Macintosh Users: We recommend using Mozilla Firefox or Safari.

Request Access to ERIS

When registering for access to ERIS, you must complete all fields with bold headings.

Data Field	Description	Purpose
M.I.	Please enter your Middle Initial	Although the M.I. is not required, it can help us identify you especially in cases of very common names
Job Title	This is your job title with your employer
Address 1	This is the primary mailing address for your employer. Please include any suite or office number on this line	After submitting the online form, PERA will send your User ID, an Agreement and Authorization for Access form to the mailing address you enter on the online form.
Address 2	If your address is too long to fit on the Address 1 line, please use this line for the street address	To ensure that the User ID letter gets to you, we must have the full mailing address.
Zip	Your 5-digit postal zip code	To ensure that the User ID letter gets to you, we must have the full mailing address.
Work Phone	Your telephone number	If problems exist with the name, address or e-mail address, we need a telephone number where we can reach you.
Email address	The e-mail address you use for your employer	Upon submission of the online form, an automatic e-mail confirming your interest in ERIS is sent to you. Also, this e-mail address is used when sending you the temporary password that is used to access ERIS the first time.
Mother's Maiden Name	Identifier for if you forget your password	If you forget your ERIS password, you must submit your User ID and the information from the "Mother's Maiden Name" field to obtain a new temporary password.

Upon completion of the required fields in the form, either tab to the "Submit" button or left-mouse button click the "Submit" button to complete the online request for access.

Once the information is received at PERA electronically, you will receive a confirmation e-mail (sent to the e-mail address listed in the online form) and then, via U.S. Postal Mail, a letter containing the User ID number assigned to you. A copy of the Terms and Conditions for ERIS use and an Agreement and Authorization for Access form will accompany the letter.

When the signed Agreement and Authorization for Access form is received at PERA, we will process your request for access and, if approved, send a temporary password to the e-mail address entered in that field of the online form. The temporary password is used to access the system the first time and will be required to be changed immediately upon use before you can enter ERIS.
